If you are ill for more than 7 calendar days, you will need a GP appointment to obtain a Med3 Form. This is a GP signed document most people call a “sick note”. See our appointments guide for more infomation about making a GP appointment.

If you recently had a sick note, and believe you require an extension, please submit an admin query. A member of our GP team will then determine if we have enough information to complete this legal document for you. If we need more information, we will invite you to make a GP appointment.

Sick note requests take time to process, so please give the team plenty of notice.
