Sick notes

If you are ill for more than 7 calendar days, you will need a GP appointment to obtain a Med3 certificate. This is also known as a sick note or fit note. See our appointments guide for more infomation about making a GP appointment.

The government provide detailed information about taking sick leave.

If you recently had a sick note, and believe you require an extension, you can request it here. A member of our GP team will then determine if we have enough information to complete this legal document for you. If we need more information, we will invite you to make a GP appointment.

Sick note requests take time to process, so please give the team plenty of notice.
