Change your contact details
Tell us if you change your name, address, phone number or email address. We use these details to send you appointment reminders, test results and invitations for checks.
How to tell us
Change your contact details onlineChoose "something else" and tell us your old and new details.
You can also tell us at reception at 54 Briarfield Road.
Changing your details in the NHS App changes your NHS login details only. It does not change the details we hold, so please tell us as well.
If you change your name
Speak to reception first. What we need depends on your situation, and they will tell you what to bring.
If you move house
If you move but still live in our practice area, tell us your new address and you can stay with us.
If you move out of our area, you will usually need to join a GP surgery near your new home. Find a GP surgery (opens the NHS website). When you join your new surgery, your records move with you. You do not need to tell us you are leaving.
Changing details for someone else
You can tell us about changes for your child, or for someone you care for if they have given permission. You can also ask for access to their health services.
Every situation is different, so please speak to reception first.
