How to ask

If you have a paper form to fill in, such as a DVLA medical form, bring it to reception. This is quicker than asking online.

For a letter, you can ask online:

Ask for a letter online

We may still ask you to come in, to give us more information or to fill in a form by hand.

Pay at reception when you collect it. We prefer card, but we also take cash.

We do this work as soon as we reasonably can, but NHS work comes first. At busy times it may take longer, or we may pause requests for non-NHS work.

What we do not do

We do not provide:

  • letters for toilet passes
  • signatures or supporting letters for passport applications
  • letters for firearm or shotgun licences
  • letters to confirm your address
  • letters about power of attorney
  • letters or forms to support immigration applications

Some forms, such as passport and blue badge applications, do not need a doctor. Another person can often sign them for free. The form will say who can sign it.

Why we charge

The NHS does not pay GPs for this work. It takes time away from caring for patients, and a doctor often needs to check your whole health record before signing, because they are responsible for what the form says.
