Send us your blood pressure or other readings
If we ask you to check your blood pressure, weight or another reading at home, you can send it to us online. You do not need an appointment.
How to send your readings
If your doctor or nurse asked you to take readings, send them using our online admin form. Say what the readings are for, and give the date and time of each one.
Send your readings onlineIf you cannot use the form, you can bring your readings on paper to reception at 54 Briarfield Road. Write your name, date of birth, and the date and time of each reading.
Urgent advice: Do not send your readings online if you feel unwell
If you feel unwell, or a reading is much higher or lower than usual, request an urgent appointment or call us. When we are closed, call 111.
Call 999 if you have chest pain, sudden weakness or numbness, difficulty speaking, or difficulty breathing.
How to take your blood pressure at home
Use a machine that goes on your upper arm. Sit and rest for 5 minutes first, and take 2 readings a minute apart. Your doctor or nurse will tell you how many days to take readings for.
