Get a sick note for work

A sick note, also called a fit note, shows your employer how your health affects your ability to work. You only need one if you are off sick for more than 7 days.

If you are off sick for 7 days or less

You do not need a sick note from us. Tell your employer you are ill. They may ask you to fill in a form to say you have been ill, which is called self-certification.

If we have already seen you about this problem

For example, if your sick note is running out and you need it extended.

Get a sick note for work

A doctor will look at your request. If they need to know more, we will ask you to book an appointment.

Please ask at least 2 working days before your current sick note runs out. A week before is better.

If we have not seen you about this problem

A doctor needs to assess you first. Request an appointment online and say that you need a sick note.

If you have been in hospital

Ask the hospital team for a sick note before you leave. They can give you one for the time they expect you to need off work.

Other notes and letters

We do not give sick notes for being off for 7 days or less. Some other letters and forms, such as for insurance or travel, are not free. See our fees for non-NHS services.
